Effective May 21, 2014 commercial vehicle drivers (CMV) can no longer get a valid medical certificate from just any healthcare provider.
CMV examinastions can only be completed by Certified Medical Examiners (MEs) listed on FMCSA's National Registry.
Suncoast Urgent Care's providers are all NRCME registered to provide comprehensive DOT physicals per the new guidelines.
GET YOUR FORMS HERE ! Make sure to download and print both forms. The first is the complete history and examination form. The second will be your certificate to bring to the Department of Motor Vehicles.
SAVE TIME , complete as much as possible at home. When you come to our office you will just have to let the receptionist know you are here for a DOT exam, you will be registered and ready to go !
